Services
- Primary Care – Preventive health, physical exams, chronic/complex disease management, medication review, diabetes education, heart healthy classes, smoking cessation, alcohol use disorder counseling, acute care, coordination of care, and case management.
- Women’s Health – Pap smears, birth control counseling, menopause counseling, intrauterine device (IUD) insertion, Nexplanon (etonogestrel) insertion, and reproductive health services. The American Academy of Pediatrics and the Centers for Disease Control and Prevention recommend offering long-acting reversible contraception (LARC) options, including IUDs and implants, with appropriate counseling and screening.
Men’s Health – Prostate health evaluation, preventive screenings, and counseling.
- On-site Diagnostics – Laboratory testing, X-ray, electrocardiogram (EKG), and low-dose computed tomography (CT) scans, as recommended by the American College of Radiology for appropriate indications.
- Musculoskeletal & Procedures – Joint injections, trigger finger injection, skin biopsies, bracing, wound care, suturing of simple and complex wounds, and other musculoskeletal procedures. Evidence supports the safety and efficacy of primary care physicians performing joint and bursa injections.
- Mental Health & Counseling – Mental health counseling, behavioral health support, and coordination of services. Structured counseling strategies such as the 5 A’s and motivational interviewing are effective for behavior change and mental health management.
- Patient Support – Patient assistance programs, medication review, and support for accessing care.
- Free heart healthy classes and diabetic classes – “insert description and patient handouts
- DOT physicals– $150 cash
