Jasmine has been a Care Coordinator at Irongate since 2021, helping patients overcome barriers that affect their health and well-being. She connects patients with community resources, assists with housing, food, transportation, health insurance, prescription coverage, DSS services, and Health Home programs, and helps coordinate behavioral health supports to ensure comprehensive, patient-centered care.
Outside of work, Jasmine enjoys hiking, biking, running, kayaking, cross-country skiing, and spending time outdoors.
Liz C., RN
Nurse Care Manager
Liz brings 45 years of nursing experience to Irongate Family Practice, with an extensive background in medical-surgical nursing, emergency care, and home healthcare. For the past nine years, she has served as a Nurse Care Manager at IGFP, where she currently works on a semi-retired basis.
Liz specializes in Transitions of Care, supporting patients following a hospital stay to help ensure a safe and seamless transition home. Her role includes following up with patients after discharge, reviewing and reconciling medications, coordinating timely follow-up appointments, answering patient questions, and providing individualized education and support. Her goal is to help patients feel informed, supported, and confident as they navigate their recovery and ongoing healthcare needs.

Care Management Supervisor
Nicole has been at Irongate Family Practice for 25 years and a RN for 15 of those. She oversees the daily operation of the care management team in helping patients improve their overall health, manage chronic diseases, and addressing barriers to care. This involves patient education, medication management, resource navigation and being an advocate for the practice’s patients.
